Integra Spine and Musculoskeletal Specialists are dedicated to serving both
you and your patients. We have specialized in the non-surgical treatment of
back pain, medical and interventional pain management for over 10 years.
We offer nearly 40 years of experience in physical therapy treatment including
spine specialized manual therapy.

Integra Spine and Musculoskeletal Specialists' clinical professionals can
determine the correct diagnosis, institute appropriate treatment and get
your patients back into work, sports and life again.

As a resource to our valued referring physicians, the adjacent section,
provided on the left hand side of the page, contains links to clinical and
technical information that may be of interest and assistance.

Referral Information:
Patients may be referred for consultation, evaluation and treatment. To make
this process a smooth transition for you and the patient, we request the
following referral form completed and sent to us via FAX at 317-823-8402.

For your convenience, you can down load a copy of our referral form to utilize,
or use your own form(s).

•  A written referral stating what you are referring your patient for.
•  All medical records pertaining to the pain problem including
    physicians' note, X-rays, MRI's, CT's, blood work and related info.
•  Patient demographics and insurance information and copy of
    insurance card if available.
•  Once this information is received, we will contact the patient for
    scheduling and to verify insurance coverage.
