How to be ready for your first appointment:
• Bring all related medical records and original diagnostic films (X-rays/MRI)
• Bring a valid insurance card and driver’s license.
• Bring a list of your current medications.
• Bring the referral from your primary care physician, if required.

During the first visit with the physician, a medical history will be
taken. The physician then reviews the history, and collects more specific information during the patient exam. In the exam, the physician may move
certain joints and limbs in ways that reveal diagnostic information about the
back or neck problem. Be sure to answer all the physician's questions
honestly because certain movements are intended to produce, or not
produce, discomfort. What you feel helps the physician diagnose your
problem correctly. Consequently, it is crucial to answer questions honestly.
For simple muscle-related back pain, X-rays are of limited value because
they do not show muscles and discs, only bones. A physician may use
X-rays to determine if there are any fractured vertebrae after a fall, or if
there is a narrowing of the disc space which may imply a disc problem.
In some cases, the physician may use MRI or CT to determine what may
be causing the person's pain. These tests are painless, and function simply
as a medical photograph of your body.
Diagnostics may include:
• X-rays show bones and the spaces between the bones. MRI (Magnetic Resonance Imaging) and CT Scans provide the physician with images of
the soft tissues and nerves in the spine, including discs and joints.
• Pain relieving injections can relieve back pain and give the physician
important information about the problem. (top)
Insurance Coverage:
Our physicians accept many HMO and PPO insurance plans and
Worker’s Compensation. This is in addition to the point-of service
and traditional indemnity insurance we also accept.
While our physicians are on many plans, an individual physician may
not be on a specific plan. You should always check when making an
appointment to ensure your insurance is accepted by the physician
that you will be seeing.
If you need to see a physical therapist as part of your treatment plan
and our therapists are not in your network, please request a list of network therapists from your insurance plan. From that list, we will gladly refer
you to a therapist we know.
If you must select a therapist outside our offices, we will make sure he
or she receives the appropriate information to treat you. If you have
any questions, ask our scheduling staff when you call for your appointment.
Once an Integra Spine physician determines your treatment plan, the plan
will be shared with your primary care physician and the referral will be
updated. If you are covered by Worker’s Compensation, we will coordinate
your coverage with your case manager or representative. (top)
Medication Guidelines:
It is important to understand that the use of many pain medicine agents
have potential complications, including the expected developments of
tolerance (reduce effect over time), dependency (the potential development
of a withdrawal syndrome upon abrupt discontinuation of opioid drugs), and
in susceptible individuals the possibility of “addiction” (wherein there is loss of control, compulsive use, and continued use despite adverse social, physical, psychological, or spiritual consequences.)
Your responsibilities while taking medicine prescribed for pain are
as follows:
1. I will only use this medicine for its intended purpose.
2. I will not take this medicine and drink alcohol.
3. I will not use machinery or drive a car or truck until approved by
my doctor.
4. I will not share this medicine with others.
5. I will notify my doctor if my illness becomes worse.
6. I am aware that this medicine may be habit-forming.
If you have problems controlling your pain with the medication prescribed,
you will need to schedule a follow-up appointment with your doctor. Violation
of published Integra Spine medication policies may lead to dismissal from th e medical practice.
