Worker's Compensation:
Integra Spine and Musculoskeletal Specialists understands the
importance of working hand-in-hand with the medical case manager, insurance claims adjuster, company representative and/or referring physicians to ensure that your Workers' Compensation patient and claimant receive the best, most appropriate and cost efficient care.
Good communication is imperative in Workers Compensation, and our clinical
and administrative staff are dedicated to ensuring that your needs are met.
When Medical Case Managers are involved in the Workers
Compensation case, Integra Spine and Musculoskeletal Specialists’
policy is to coordinate care through the Case Manager. This means
that our office works with the case manager to set up the initial appointment,
fax copies of medical records to the case manager, refer the patient to the
case manager if the patient calls the office to cancel or change their
appointment, notify the case manager of prescription or treatment changes,
or in case of No Shows for scheduled appointments or treatments.
We have specialized in the non-surgical treatment of back pain, physical
medicine & rehabilitation, and medical and interventional pain management
for over 10 years. Plus, we bring over 30 years of experience in physical
therapy treatment including spine specialized manual therapy. Therefore,
Integra Spine and Musculoskeletal Specialists can determine the correct
diagnosis, institute appropriate treatment and get your patient back to work
and close your case as soon as is possible.

Referral Information
Patients may be referred for consultation, evaluation and treatment.
To make this process a smooth transition for you and the patient, we
request the following information, via fax at 317-823-8402. For your
convenience, you can down load a copy of our referral form to utilize,
or use your own form(s).
• A written referral stating what the patient is being referred for.
• All medical records pertaining to the neuromusculoskeletal problem including physicians' note, X-rays, MRI's, CT's, blood work, etc.
• Complete medication list.
• Patient demographics, insurance information and copy of insurance card if available.
• Workers Compensation authorization for treatment number.
• Claim number and date of injury.
